Collection of your personal information Registering as a service user:When you register a young person with us, we collect information on the following:
- name
- home address
- age
- gender
- medical conditions that are relevant to their care at our sessions (includes allergies, dietary requirements or medical condition)
- name, address and contact number for parent/guardian and emergency contacts should we need them.
We ask for this information in order to contact you and your young person about our provisions and also to use in the event of emergencies.
Registering for our mailing list:If you sign up to the newsletter, we will collect the following information:
- Name
- Email address
This information is used to keep you informed about ESY news and events. You are able to unsubscribe from this list any time be clicking “unsubscribe” or emailing the ESY office. Your details will then be removed from our mailing list.
Use of your personal informationYou can be assured that:- we do not sell, rent or lease customer lists to third parties
- we only send you emails relating to your young person and any activities they may take part in.
- if you are a subscriber to our email list, we will only contact you to send our quarterly newsletters and to inform you of any events that are taking place. You can unsubscribe from this at any time.
- we never disclose personal information without your explicit consent unless required to do so by law (see below)
